FOR ALL LADY DI CUSTOMERS

Please read over and complete the enclosed questionnaire before your pet’s interview and evaluation. This form will remain on file for any future visits.  Before your pet’s care by Lady Di, a free evaluation must be completed.

Hours of operation are Monday-Thursday 7AM-8PM, Friday 7AM – 7PM, Saturday 9AM-7PM & Sunday 10AM – 6PM. Please plan your drop off and pickup times accordingly, as these times will be strictly enforced.  Phone hours are 8AM – 6PM.  No drop offs or pick-ups on Thanksgiving, Christmas Day & New Years Day.  Exceptions in an emergency only.

Reservations for care during the holidays must be made 1 week in advance – this includes: Memorial Day, 4th of July, Labor Day, Thanksgiving, Christmas & New Years.

Appointments for interviews/evaluations are scheduled Monday through Saturday from 10AM – 7PM.  Please plan on spending one half-hour with us to discuss your pet’s habits, preferences or special needs.

After the initial evaluation, visits may be arranged by phone @ 206-245-0656 or by e-mail at ladydipets@comcast.net. Check-in for these visits usually takes about 10 minutes.

Payment is due upon pickup.  Please make checks payable to Lady Di. Payment is due in advance if someone else is picking up your pet from Lady Di’s.

FOR DOG CARE IN LADY DI FACILITY

For the safety of all pets we require proof of all vaccines at the time of the initial consultation. We also require that you present proof of inoculation against DHLPPC and Bordetella. Sorry, but if we cannot verify shot records you will be asked to reschedule your appointment. You will be asked when you call in to schedule subsequent appointments for updated vaccination information.  Fecal exam is required yearly – proof from vet is required.

We accept dogs of all sizes after an evaluation. We prefer if dogs are neutered or spayed. Exceptions will be made.

All dogs must wear identification tags at all times.

Please provide enough food for your pet’s stay and mark all containers with your name.  Additional charges will apply for food supplied by Lady Di.

We require that all dogs be on an effective flea control regiment, such as Program, Frontline or Advantage.  All dogs will be physically inspected for fleas and/or ticks before they are brought into the Lady Di facility. No infested pets will be accepted.

Lady Di accepts Cash, Checks, and all major credit cards.
